In consideration of services to be received as a member of the bjjcoach.com website, the person agreeing releases and forever discharges Marcello C. Monteiro and the website bjjcoach.com from any and all actions, causes of actions, liability, claims and demands upon or by reason of any damage, loss, injury, or suffering, whether known or unknown or which may be sustained by the applicant in connection with and in the course of receiving Martial Arts instruction at the website bjjcoach.com, from the instructor(s), staff, officials, or employees of the bjjcoach.com website or any fellow students from the website in connection therewith and within the course of taking training or lessons. He/she hereby waives all his/her rights to the claims, actions, demands or suit for loss, injury, damage or suffering sustained in the aforementioned manner and circumstances. The undersigned assumes all risk inherent and incidental to this type of sports activity a condition to applying for admission to the website bjjcoach.com. In consideration of all the benefits indicated in this document, the undersigned agrees to hold Marcello C. Monteiro and the website bjjcoach.com harmless from and against all claims, demands, actions, cause of action or suits which may be brought against Marcello C. Monteiro or the website bjjcoach.com by anyone associated with student, school, website, parent, relative, on account of injury or damages or suffering inflicted upon him/her in connection with lessons within or without of the school property. I affirm that I have no medical condition which would be either a hazard or would create a medical impairment upon student named. Student/ responsible person will make sure that all protective gear is worn during training. I do hereby affirm that am at least 18 years of age and that I execute this document voluntarily and with full knowledge of its significance. Any change in this agreement may only be made if both you and Marcello C. Monteiro agree in writing to any such change.
Hold Harmless Agreement
Upon being permitted to use the website bjjcoach.com to learn Braziian Jiu-Jitsu techniques I do hereby release Marcello C. Monteiro and or any staff or instructors from any liability or any injuries or death incurred during training, online classes, or other activities sponsored by Marcello C. Monteiro.
Furthermore, I understand that martial arts training consists of some physical contact which requires students to exercise extreme caution at all times. I will not engage in any unauthorized contact practice nor will I in engage in contact practice without proper protective equipment ( headgear, hand pads, foot pads, mouth piece, groin cup, etc.). I will avoid horseplay and other acts that could cause needless injury for which I could be held responsible. If any clause, sentence, phrase, or statement is found unenforceable or invalid by a court of law the remainder of this agreement shall remain valid and enforceable and any invalid clause, sentence, phrase, or statement shall be considered struck from the agreement. I declare that I am of sound mind and body upon agreeing to the terms of this agreement and understand that this agreement may be used in a court of law for any claims against Marcello C. Monteiro, the website bjjcoach.com, and the MCMBJJA. I have read this agreement and understand the content of it and agree to abide by the terms of it.
Sales
All sales made on bjjcoach.com are final. All refunds and reimbursements are givin at the concent of Marcello C. Monteiro. All purchases reguarding the paid preium memberships are held on automatic rebilling cycles. Therefor, at the end of the term, you will be rebilled for the same amount as before, and will recieve the same amount time purchases previously. This will happen for a total of 3 years of subscribes services. You may cancel you subscription at any time by clicking the "My Subscriptions" link on your user menu and canceling your membership.
